If someone has a great deal of sun damage, if they have smoked a number of years, if they have abused alcohol, or if they are 65 or older, then they may not be able to achieve as dramatic a change as they might wish. Also, if you have very fair Scotch Irish complexion, then the skin has a usually thinner dermis (this is the thick elastic part of the skin), and this will have less elasticity and show wrinkles more quickly with time. Therefore, these patients usually do not get quite as good a result or as long lasting a result. The lift will hold up well, but the skin will tend to form lines and redundancy more quickly.
Also, as we age, part of the aging process is a decrease in blood supply to all parts of our body, including our face. Our skin becomes thinner and we have less thickness and elasticity in the dermis. But one of the key things in aging, that most people don't recognize, is that our faces deflate. That is, as we age, we tend to lose our facial fat and become more gaunt in appearance. If you look at a young person's face, it is full, soft and round. This is why that removing while it's perfectly okay to remove fat in the neck and submental (underneath the jaw) area, you never wish to remove fat in any significant amount in the face itself. Facial fat is precious and something that will continue to decrease as we age. One of the mistakes that plastic surgeons made a while back was to remove buckle fat. This is a very specialized fat pad in the cheeks and surgeons were removing it from a small incision inside the mouth in order to give a more defined cheek bone look and a less round face look. The problem is, that this improvement may look good for 5 or 10 years, but then for the next 30 or 40 years, you will have a hollowed cheeked, gaunt, unattractive look. So, removing buckle fat is indicated only in a very few people say most surgeons.
