Are You Ready to be Consolidated: Half of all Practice Managers Will be Obsolete in the Next 5 Years

Posted: Dec 06, 2010 |Comments: 0 |

The most radical transformation of the healthcare system is about to take place, and most practice managers are not ready for it. Yes, most are feeling the pinch of declining revenues in their practice, but few have any idea what is about to hit them. …its called "consolidation", "vertical integration", "accountable health organization" and numerous other newly minted names. What it means for practice managers is fewer jobs!

I have recommended that all physicians read the recent article posted in the Annals of Internal Medicine.  It spells out the Obama administration's design for the new healthcare system. This also is a MUST READ for all practice managers. In a nutshell, healthcare reform will usher in a new reimbursement system called "outcomes based, bundled billing". Physicians will be organized into large vertically integrated groups and will offer "coordinated care" for certain disease conditions. Rather than each physician billing for their services, the group will receive a "bundled fee" based on the overall care and outcome of the case. Each physician involved in the case will get some share of the pie, depending on the contracted arrangement with the "vertical organization".

The idea of a "vertical organization" is to provide more coordination of care and payment based on successful management of the case. Also, it is intended to reduce the inefficiencies and waste produced by un-coordinated care from multiple doctors in small practices.

Do the math. If, for example, twenty-five practices of various specialties are organized into a vertical organization, what do you think will happen to the administrative staff of each practice. The whole idea is to cut cost and waste. That means fewer employees and managers at the home office. If you doubt this prediction, just ask what happened to the practice managers of practices that have been purchased by hospital systems.

This may sound like doom and gloom for all practice managers, but it is not. There will be great opportunities for those practice managers who can help launch their practice into the future. There is no time to wait. The wheels of change are in motion.

In Part I of our blog series, we explained the "New Rules" for physician compensation based on the post- reform healthcare system. As we stated, physicians are at the epicenter of one of the most dramatic periods of change in America's healthcare system. The rules and strategies for physician success are changing rapidly, and most physicians are struggling to understand it.

I'm not a gambling man, but I would venture to place a confident bet that the majority of patients in this country have never taken a good look at their personal medical record. If they did, they might be surprised to find the gaps in information that their primary care doctors rely on for their diagnoses, treatment and long term care plans.

It is common knowledge that the global economy has been irreversibly changed since last Summer. Now nearly two years into serious recession, the United States has scrambled to hold any sort of ground in consumer confidence, losing countless jobs and huge revenues in almost every sector. Real estate and retail have been hit especially hard, forcing bankruptcies and driving up.. The one sector of the economy which has held steady in both job creation and econoic output, however, is health care.

The key reason why some firms thrive while some implode during an financial recession is still a puzzle to many people business-owning business owners. Some wrongly assume that all businesses should suffer via recessionary cycles. But the truth is that some companies are usually essentially recession-proof, and it is not necessarily because they are much larger, better known, or a lot more generously capitalized.
