An insight is although earth was created around 4.5 billion years ago, life began to exist not long after including the nasty smelly breath bacteria. As you all know (I hope :-) all life is formed from cells, and all life traces back to a single primordial twitch from which all life evolved from, so you see our line of life is thousands of millions of years old. The pertinent fact is all life is one, along with the bacteria in our mouth.
We all have bacteria in our cells, we could not live without them, yet they communicate in different languages, it's like they're not sure about the relationship, they still have their bags packed, ready to go (and for smelly breath sufferers it couldn't be too soon).
Having discussions like this lets us drill down inside our preoccupation with Bad Breath as sufferers. It may end in a result, possibly in halitosis sufferer's favour! Though probably not. We didn't cross the border into Halitosis the border crossed us.
You see smelly breath is only a problem in today's modern paradigm of our concentrated social existence.
If we follow this line of thought we may very well discover that permanent lifelong Bad Breath is historically completely normal, you may even find those who do not suffer smelly breath are the odd ones out.

It's likely bad breath is exactly what our biological make up intended from the beginning. Will we evolve to a halitosis free existence? Probably Yes- due to fresh breath people having more babies' then smelly breath people. How long will this take? Just look at the sex drive of modern day humans, it's like we are still fighting extinction as our hormones have yet to realise humans number in the billions. So fresh breath for all won't be anytime soon.
A good test (not for all) is to lick the inside of your wrist and smell it 15 seconds later. No smell is good but you can still have bad breath. Bad smell is bad, you can safely times the smell by ten and you'll get an idea how your breath smells to others.
Think about this; a fast moving stream results in clean fresh water, but very slow moving or not moving body of water will quickly turn putrid and become full of bacteria. You see It's the same for our mouth, more saliva and more liquids results in a clean fresh oral environment. Slow moving saliva or little or no saliva will quickly turn putrid and full of bacteria. Thus resulting in bad to absolutely disgusting breath.
In the mean time if you suffer from smelly breath-first floss then intensely brush not just your teeth but your gums, cheeks, roof of mouth and importantly your tongue. Only drink sugar free liquids and plenty of them. Exercise daily, get plenty of sleep each night, see your dentist twice a year and tell him your problem and follow his advice (if any).
