A breast implants (also known as breast augmentation) operation usually involves placing of an artificial implant either under breast tissue, or under chest muscle behind the breast.

Why have breast implants operation?

The reasons why a patient may choose to enlarge her breasts include:

·Feeling that her breasts are too small

·Loss of breast volume after pregnancy (although implants will not lift them)

·Difference in size of the two breasts

·Reduced breast size after losing weight

Breast implants surgery may also be offered if patient is having surgery for breast cancer or other conditions that may affect the size and shape of her breasts.


Types of Breast Implants:


There are two basic types of implants: saline filled and silicone gel-filled breast implants. Both types have the same basic type of outer envelope made up of a solid, rubber-like form of silicone called an elastomer.


·Saline filled implants – These implants are made of an elastomer silicone envelope, which is surgically implanted under the tissues, and then filled with sterile saline, a salt-water solution, through a valve.

·Silicone gel-filled implants – These implants are made of an elastomer envelope pre-filled (prior to surgery) with a clear, sticky, thick jellylike form of silicone that approximates the consistency of breast tissue. Silicone gel implants come in many sizes. The size used depends on the amount of augmentation desired or the size the reconstructed breast is to be.


What's the difference between the two types of breast implants?


Each type of breast implant is filled with a different material. Saline-filled breast implants consist of a silicone shell that's either pre-filled with saline or filled with saline at the time of implant surgery. Silicone gel implants also consist of a silicone outer shell, but they're filled with a silicone gel rather than saline.

Silicone gel has viscosity- thick, sticky fluid consistency that closely mimics human fat, which is a large component of female breast tissue. Saline, on the other hand, isn't compressible and gives a more firm impression.


Implant placement:

Subpectoral or submuscular implants:

Placement under the pectoralis muscle is the approach most commonly used. It has a lower risk of capsular contraction, interferes less with a mammogram, and a better cosmetic result in women with small breasts. It is associated with a lengthier recovery and more pain and swelling than placement above the muscle. This approach requires general anesthetic.

Implants that are placed above the pectoralis muscle and below the breast tissue. This has a greater risk of capsular contraction, interferes more with a mammogram. It has a better cosmetic result in athletic women (body builders may want to consider a male pectoral implant as it can look more natural). It is associated with a shorter recovery and less pain and swelling than placement below the muscle. This approach can be performed with intravenous sedation and local anesthetic.

Incision Sites:

Inframammary incision:

The incision is made on the lower portion of the breast, in the crease where the breast meets the chest, so that any scar will still be hidden. This incision allows the surgeon the best visibility during surgery. It is the most commonly done approach.

Peri-areolar incision:

The second most common incision is made in the areola. The incision is usually a small semi-circle. The scar is camouflaged by the nipple. However, if there are any imperfections in the scar, it will be highly visible. This type of incision has an increased risk for diminished nipple sensation.


Axilliary incision:

The incision is made in the armpit, and may require the use of an endoscope. The scar is well hidden, but provides poor visibility for the surgeon.


Umbilical incision:

The incision is made in the belly button. It usually requires the use of an endoscope, and it is very difficult to place the implants below the muscle utilizing this method.
