A Crohn's patient with low sodium 132, low potassium 3.0, low chloride 90, high RBC 551.  Is it critical he gets IV fluids at this point, or can he wait and try to improve on his own, as long as potassium level does not fall below 3.0? At what point does it affect the heart and kidneys?  In the past, with sodium and potassium a bit lower, he has had to be rushed to the hospital in severe dehydration, elevated heart rate, low blood pressure, leading to heart fibrilation, and kidney failure.  He does not want to go to the hospital yet.  I am inclined to think he should,  before it gets any worse, as before.
