Getting to Know the Pill

Of all the contraceptive methods we've discussed in this series, none is more effective and more controversial than the pill. Next to the condom, the pill is perhaps the most misunderstood contraceptive in the market. But that hasn't stopped it from being the most popular at the same time.

In America, over 13 million or one out of four women below 45 regularly uses the pill. Worldwide, there are over 60 million women on the pill.

The search for an effective oral contraceptive goes back centuries ago. In the early days, women took arsenic, a highly toxic compound, and strychnine or rat poison to prevent pregnancy. Most of them probably died from taking these poisonous substances.

Later, an important discovery was made: ovulation (the release of an egg from one of the woman's ovaries every month) does not occur during pregnancy. This simply means that a pregnant woman cannot get pregnant again until after she gives birth. This is due mainly to the action of certain hormones in the ovaries which prevent the release of the egg.

In the late 1920s, researchers learned that there are two hormones which control ovulation namely, estrogen and progesterone. Synthetic versions of these hormones were eventually developed and they became the active ingredients of the modern birth control pill.

What does the pill really do? It creates a state of pseudo-pregnancy which interferes with the delicate balance of hormones produced in the brain and ovary during the menstrual cycle. By suppressing the body's natural hor­mones, the pill stops the ovaries from releasing eggs.

"Moreover, the pill also prevents the cervical mucus from becoming watery, as it normally does during ovulation. Instead, the mucus for the entire cycle is scant, making it difficult for sperm to reach the fallopian tubes," said Dr. David E. Larson, editor-in-chief of the “Mayo Clinic Family Health Book.”

There are many types of pills. The combination pill contains both estrogen and progestin, while the mini-pill only has progestin. They work the same way but the combination pill is more effective. However, for women above 35 and those with problems like hypertension and diabetes, the mini-pill is safer.

“By contrast, extended-cycle birth control pills contain active hormones for every day of the month. The newest extended-cycle regimen, Lybrel, is a low-dose pill that's designed to be taken continuously for one year, with no breaks for hormone-free intervals. Lybrel is meant to suppress all menstrual bleeding,” explained the editors of the Mayo Clinic.

“Two other specially designed, extended-cycle pill regimens - Seasonale and Seasonique - are also currently on the market. With both pills, you take active hormone pills every day for three months, then you take one week of placebo pills (Seasonale) or low-dose estrogen pills (Seasonique). You'll experience menstrual bleeding during that week,” they added. (Next: Problems with early pills.)
