A kick serve is a great change up when you want some variety on your first serve, but it's mainly used as a second serve because it can be controlled much easier than a flat first serve. To be able to hit a kick serve effectively, it must jump up high from the bounce and beat your opponent through height and placement, rather than speed.
To hit a kick serve you must swing your racquet so the strings brush from low to high over the ball. This will be hard to get right at first, but creating very fast racquet head speed is key to make this work. Swinging your racquet very quickly up the back of the ball brushing the strings will create a lot of spin and kick when it hits the ground.
The ball toss should be thrown slightly behind you and to the left. This might not be possible if you have a bad back, but the more leverage you can get out of your body the better. The consistency of your kick serve depends on the consistency of your serve motion.
Keep in mind that you want to be aiming to clear the net by about a meter, since the spin from the serve will naturally bring the ball down shorter than a ball without spin.
Once you've mastered getting this serve in with lots of kick on it, start practicing your accuracy. Variety on serve is incredibly important and keeping your opponent guessing about what second serve is coming next with serve you very well in your matches.
